Pharmacists - Medication Therapy

The role of the pharmacist in the delivery of healthcare today is changing. Clinical pharmacists are taking an active role in monitoring the care of customers. Medication Therapy Management (MTM) is currently being reimbursed by many health insurance plans.  In order document your interventions effectively, Healing Our Village, Inc. has developed WellTrak™.

Healing Our Village, Inc. clinical pharmacists will provide the following services for all patients with drug therapy problems via our web-based consultation program

  • Evaluate the medication therapy in all patients with a diagnosis of diabetes mellitus.
  • Alert the primary care provider when an individual is not at an hemoglobin A1C value of less than 8%
  • Evaluate, and assist the primary care provider, in monitoring whether the diabetes quality measures (blood pressure, A1C, LDL-C, eye examinations, foot examinations) have been ordered and evaluated on the patients with diabetes.
  • Evaluate patients for the presence of high risk of adverse outcomes from medication use, including drug interaction and drug side effects.
  • Monitor and encourage compliance or adherence to prescribed medications.

Provide a detailed review of medications in patients who are experiencing adverse   outcomes and recommend drug regimen changes to the primary care provider through written consultations
