Comprehensive Diabetes Education
Education and training for individuals with pre-diabetes (strong family history of diabetes with elevated blood sugars) and/or type 2 diabetes to assist them in developing their own management plan. Participants will gain a better understanding of the diabetes, risk factors for developing diabetes, measuring blood sugar at home, and preventing, detecting and managing diabetes complications. Instruction will stress the importance of diet, exercise and stress management, review of diabetes medications including insulin administration. Ages: 13+
“Waiting to Exhale”
Asthma Management
Education and training for individuals with Asthma. Participants will gain a better understanding the warning signs of asthma, and how to control it and prevent attacks. In addition, they will be given information needed to empower them to make decisions about managing their asthma through the use of symptom/trigger identification and control, measuring the severity of attacks, and the use of asthma medications. This interactive program will include medication review and instruction on the use of peak flow meters. Ages: 13+
“Lifestyle Choices and Changes”
Risk Factor Reduction
Education and training for individuals committed to changing unhealthy lifestyle behaviors, including eating patterns leading to obesity, smoking and lack of physical activity. A review of the prevention and management of cholesterol and hypertension will be provided. Ages: 13+
Nutrition component – will help patients increase understand how to make healthy food choices, how to cut fat and empty calories from their diet, how to get the proper nutrients, how to read food labels, how to prepare foods in a healthy way and how to make healthy food selections based on a limited budget. Emphasis also will be placed on how to increase physical activity and incorporate exercise into your daily life. Ages: 13+
Smoking cessation component - will help the member to gain more insight into: the main reasons they smoke, why they want to quit, how to deal with the events or situations in their lives that may lead them to smoke and the best stop smoking method for them. The program is designed so that the member will begin the five step program for quitting. Individual session can be requested to help the member implement their stop smoking plan, provide support as they experience nicotine withdrawal and instruction about how to deal with those situations during which they would normally smoke. Ages: 13+
Exercise component - will teach the participant a low impact aerobic exercise routine that can be done at home. The goal of the program is to increase the leisure time activity level of the participant using the American Heart Association “Choose to Move”. Ages: 13+
"Positive Living "
HIV Health Education
This program is for participants that are contemplating sexual activity or are currently sexually active. The program will educate the participant on various high-risk sexual behaviors that lead to sexually transmitted diseases like HIV/AIDS. The participant will understand the difference between HIV and AIDS, blood tests that are used to assess severity of the disease (CD4 and viral load), importance of taking mediations as prescribed, common drug interactions, how to reduce drug resistance, treatment of other infections and the goals of treatment. Ages: 13+

Healing from the inside out –
A Behavioral Management Workshop
This workshop will give participants an overview and definition of depression, anger management and alcohol/substance abuse. Through an interactive format, participants will learn to recognize the symptoms of and the relationship between these three topics as well as effective coping strategies for daily living. Ages: 13+
